Please be advised that our friendly front office staff will request your insurance card and photo identification at time of check-in. Also, please be advised that your insurance card will be copied for billing purposes.
Payment of insurance co-pays, deductibles, and non-covered services must be made at time of service. Payment can be made by checks or credit cards. We do not accept cash payment. Primary and secondary insurance provided at time of service will be billed as courtesy on the behalf of the patient. All balances remaining after insurance payment will be billed to the patient within 30 days.
